Guest Questionnaire

Dear Guest,

I was wondering if you would take part in a survey I am conducting about the bipolar guests that visit my site.  I am researching the type of bipolar sufferers who come to this site and what they are looking for, so that I can better provide for them at "My Mental Trampoline".  All information is confidential and I do not want to know your names, addresses etc.  Any information collected will not be shared with other sources.  I just want to know what kind of articles and information would best suit the people that come to read and post at my site and message board.  I would appreciate your participation and thank-you for the time you took out of your day to complete this questionnaire.  Take care and good mental health,

                                                                                                        Tatty
